• 2nd Step: Our experts will then identify the properties that can fulfill the referral criteria of the participants, their support needs and compulsions, location, and budget. Once done we will send the quote to the referrer.
• 3rd Step: Now the referrer will confirm the NDIA approval for proceeding and will request for a SDS Care service agreement.
• 4th Step: This step involves the signing of the agreement and liaison with the referrer and the team for coordinating delivery and access of equipment.
• 5th Step: Final confirmation that will pave the way for a seamless transition and arrival of the participant. This will follow feedback and monitoring for transition assistance for the participants and support.
