How We Can Assist With Your Hospital Discharge

Planning for Discharge: The discharge process usually begins soon after admission and involves a team of healthcare professionals. They develop a discharge plan based on the patient’s medical condition, recovery progress, and personal circumstances. The goal is to ensure that the patient can
safely leave the hospital and continue their recovery elsewhere. We take part in meetings and planning processes.

Education and Training: The patient and their caregiver are often educated about their post discharge care. This can include instructions about medications, physical activity, diet, wound care, follow-up appointments, and when to seek emergency care.

Coordination with Post-Hospital Care Providers: If the patient requires ongoing care after discharge, we assist with support such as nursing and Allied Health. We ensure a smooth transition and continuity of care.

Discharge Summary: The hospital provides a discharge summary, a document that includes information about the patient's hospital stay, treatment, and recommendations for follow-up care. This is typically shared with the patient’s primary care doctor and other stakeholders.

Follow-Up Care: We assist with after-discharge, as the patient often has follow-up appointments
with their doctor or specialists to monitor their recovery.

Evaluation and Adjustment of Discharge Plan: The discharge plan is usually a flexible document that can be adjusted based on the patient’s recovery progress and needs. It’s evaluated regularly, and changes are made as necessary.
