How Our MTA Process Works

  1. Step 1 – Submit Your Referral: Complete and send the NDIS referral form. Our SDS Care Specialist and Navigator will contact you to discuss your accommodation needs.
  2. Step 2 – Property Matching & Quoting: We identify suitable properties based on your care needs, location preferences, accessibility requirements, and budget. A quote is then provided to the referrer.
  3. Step 3 – NDIA Approval: The referrer confirms NDIA funding approval and requests an SDS Care service agreement.
  4. Step 4 – Agreement & Setup: We finalise the service agreement and coordinate with the referral team to ensure equipment access, support delivery, and transition logistics.
  5. Step 5 – Arrival & Ongoing Support: Participant transitions into the accommodation. We provide ongoing support, feedback opportunities, and transition monitoring to ensure stability and satisfaction.
